Financial and Refraction Policies

Insurance:

When making an appointment with Dr. Carter, it is your responsibility to confirm your benefits and plan requirements with your insurance company. If your plan requires a referral and you or your Primary Care Physician do not provide one, please be prepared to pay for your visit in full or reschedule.

Late arrivals:

We do our best to keep to our schedule. If you arrive more than 15 minutes past your scheduled appointment time, you may be rescheduled so that other patients are not inconvenienced.

Check-In:

NEW PATIENTS: Please arrive 15 minutes prior to your scheduled time so that all paperwork may be completed before you go in to see Dr. Carter.

You must present your current insurance card along with a valid picture I.D. Without your insurance card, we will be unable to file your insurance therefore the balance will be your responsibility.

Co-pays and balances are due at check-in time. We accept payments in the form of cash, check, Visa, Mastercard and Discover and American Express. At each visit, you will fill out a short patient demographic and present your insurance card so that our records will stay up-to-date. You must inform our office of any changes.

No Show / Missed Appointment Policy

In order to provide the best care and service to our patients, we ask that you notify us 24 hours in advance to cancel and/or reschedule your appointment. This is so we can accommodate our other patients and ensure the missed appointment does not interfere with our plan of treatment for you.

  • A 24 hour cancellation notice is required. If we do not receive this notice, a $20 fee may be assessed to your account.
  • After 2 missed appointments (failure to show or call), you will be required to discuss your missed appointments with Dr. Carter prior to scheduling a new appointment.

Refraction Fee:

Refraction is the process of determining the eye’s refractive error or need for corrective spectacle and/or contact lenses. Dr. Carter also does this for diagnostic purposes. It is an essential part of an eye examination, but it is NOT covered by Medicare or most insurances. Our office fee for a refraction is $20.00

We will always file this to your insurance first. This does not need to be paid upfront.
